Dr. Note is an AI-powered dental chart note assistant ("the Service"). The Service helps dental professionals generate clinical documentation based on templates, rules, and brief descriptions they provide. Dr. Note is a documentation tool — it does not provide medical advice, diagnoses, or treatment recommendations.

2. Professional responsibility

You are solely responsible for reviewing, verifying, and approving all AI-generated content before including it in any patient record. The Service may produce errors, omissions, or inaccuracies. As the treating clinician, you are the custodian of health information and bear full professional responsibility for the accuracy and completeness of your chart notes.

3. Acceptable use

By using Dr. Note, you agree to:

→ Not enter patient names, health card numbers, or other personal identifiers

→ Review all generated notes before using them in clinical records

→ Use the Service only for legitimate dental documentation purposes

→ Comply with all applicable privacy laws (PIPEDA, PHIPA, etc.) in your jurisdiction

→ Keep your account credentials secure

4. Subscription & billing

Dr. Note offers a free trial of 30 notes. After the trial, continued use requires a paid subscription at the current rate ($49 CAD/month as of July 2026). Payment is processed by Stripe. You may cancel at any time; cancellation takes effect at the end of the current billing period. No refunds are issued for partial months.

5. Data & privacy

Your use of Dr. Note is also governed by our Privacy Policy, which describes how we handle data, support PHIPA/PIPEDA compliance, and protect your information.

6. Disclaimer of warranties

The Service is provided "as is" without warranties of any kind, express or implied, including warranties of merchantability, fitness for a particular purpose, or accuracy of AI-generated content. We do not guarantee that the Service will be uninterrupted, error-free, or meet specific clinical standards.

7. Limitation of liability

To the maximum extent permitted by law, Dr. Note and its operators shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits or revenue, arising from your use of the Service. Our total liability shall not exceed the amount you paid for the Service in the 12 months preceding the claim.

8. Termination

We may suspend or terminate your access to the Service at any time for violation of these terms. You may delete your account and all associated data at any time from the Account panel.

9. Changes

We may update these terms from time to time. Continued use of the Service after changes constitutes acceptance. Material changes will be communicated via the app or email.

10. Governing law

These terms are governed by the laws of the Province of Ontario and the federal laws of Canada applicable therein.
